Germanium is a metalloid in group 14 of the periodic table, sitting between silicon and tin, and it shares the tetravalent bonding chemistry of that group.
No essential biological function has been established for germanium in humans; there is no recommended intake, no known germanium-dependent enzyme and no described deficiency state.
Germanium occurs in the diet at trace concentrations reflecting soil content, with foods contributing microgram-level amounts.
The organogermanium compound Ge-132, bis(2-carboxyethyl)germanium sesquioxide, hydrolyses in water to 3-(trihydroxygermyl)propanoic acid, so the sesquioxide is a solid-state form of the same species in solution.
Where Germanium comes from.
Nobody digs for germanium on purpose. It comes out as a leftover from zinc smelting and from the ash of certain coals, gets cleaned up as a gas and turned into a white oxide powder, and that oxide is then chemically joined to a carbon chain to make the organic form used in supplements. How much of the plain inorganic oxide is left behind is the number worth asking about.

Germanium is not mined for its own sake; it is recovered as a byproduct of sphalerite zinc smelting and from the fly ash of certain germanium-bearing coals.
The concentrate is treated with hydrochloric acid to form volatile germanium tetrachloride, which separates it from the bulk of the ore matrix.
Germanium tetrachloride is purified by repeated distillation, then hydrolysed with water to precipitate germanium dioxide.
Germanium dioxide is reduced and reacted with acrylic acid, or the corresponding trichlorogermane route is used, to form the germanium-carbon bond of the sesquioxide.
The organogermanium product is recrystallised to reduce residual inorganic germanium, which is the impurity of consequence in this material.
Total germanium is measured by ICP-MS, with a separate specification limiting residual germanium dioxide; the two numbers are not interchangeable.
The purified compound is dried, milled and blended for capsules or tablets.
Product labels typically declare elemental germanium content without stating the residual inorganic germanium level, and the two are different measurements.
