- Nobody has measured a time course for swallowed mistletoe in people, so there's no honest duration to give.

Why these belong in the same formula. Each row says what the basis is, from settled biochemistry through to a trial that measured the pair.
Garlic has reasonably consistent data for modest reductions in blood pressure, and mistletoe preparations have a traditional and mechanistic association with the same direction of effect. Stacking two agents that push the same way is worth flagging in anyone already on antihypertensive medication. This is a caution, not a recommendation.
Nitrate to nitrite to nitric oxide conversion produces a measurable drop in blood pressure in most people. Combining that with a botanical carrying its own blood-pressure association means the two add. Anyone whose blood pressure already runs low, or who is medicated for elevated blood pressure, should raise the combination with their clinician.
Astragalus polysaccharides and mistletoe lectins both act on immune cell signalling, though through entirely different molecular routes. Combining two immune modulators is not automatically additive and can pull in opposing directions. Anyone taking immunosuppressive medication or living with an autoimmune condition should not combine them without medical oversight.
Turkey tail acts largely through beta-glucan recognition by dectin-1 and related pattern receptors, while mistletoe lectins bind galactose residues on cell surfaces. Two different recognition systems, both pointed at innate immune signalling. The combination is theoretical and has not been characterised, so it belongs in the mechanistic column only.
The pairing is a practice pattern in the clinics that use injectable mistletoe extract, not a documented pharmacological interaction. Naming it here is about describing what actually happens in the field so the page reflects reality. It carries no implication that the combination does anything the individual components do not.
Viscumin and the other mistletoe lectins are large glycoproteins, and proteolysis in the stomach and small intestine breaks them down like any other dietary protein. This is why the clinically studied preparations are injected rather than swallowed. Added protease activity moves in the same direction, and it is the single most important thing to understand about an oral mistletoe product.

European mistletoe has three main active ingredient groups, ribosome-disrupting proteins called mistletoe lectins, small toxic peptides called viscotoxins, and a fraction of flavonoids and phenolic acids.
One of the lectins works by first latching onto sugar molecules on the outside of a cell, then getting inside and damaging the cell's ribosomal rna, which shuts down protein production in that cell.
Because these are proteins, stomach acid and gut enzymes break them down, so taking them by mouth can't get them into your bloodstream intact. the preparations that have actually been studied clinically are given by injection, under medical supervision.
What's actually in a mistletoe preparation depends heavily on which tree it grew on, when it was harvested, and whether it's a summer or winter extract, which is why traditional versions are named after the host tree.
