White heather and uva ursi both contain arbutin, which passes largely intact to the urine before microbial and enzymatic hydrolysis releases hydroquinone. Combining them raises total arbutin intake from one pathway rather than two. That also means the ceiling on how much hydroquinone exposure is reasonable applies to the pair together, not to each separately. No human trial has tested the two side by side.
Arbutin derived compounds behave differently depending on urinary pH, and the traditional practice was to keep urine on the alkaline side rather than acidify it. Ascorbic acid taken in gram quantities pushes urinary pH down. Anyone pairing the two is working against the conditions the traditional use assumed. This is pharmacokinetic reasoning, not a measured clinical outcome.
Polyphenol rich plant infusions form insoluble complexes with ferrous and ferric iron before absorption can occur. The effect is on non-heme iron from food or from an iron salt, and it is dose related to the tannin load. Separating an iron dose from a heather infusion by a couple of hours sidesteps most of it. Heme iron is much less affected.
Heather is rich in quercetin type glycosides of its own, so a separate quercetin dose adds to an already loaded pool. Lactase phlorizin hydrolase and the UGT and SULT conjugating enzymes that handle these flavonoids are saturable. In practice the combination gives less than additive plasma exposure. The direction of the interaction is understood, the size of it in people is not.
Heather appears in old European formularies alongside dandelion and other plants used to increase urine volume. The rationale was dilution and flushing rather than any named pharmacology. Both plants also carry potassium in meaningful amounts, which is worth noting for anyone already on a potassium sparing agent. Read the pairing as convention, not as tested pharmacology.

Arbutin passes through the stomach mostly unchanged, gets absorbed and processed by the liver, then leaves the body in urine as modified forms, though some free hydroquinone can be released locally along the way.
Tannins in heather bind and precipitate proteins on contact, which is what gives a heather tea its drying, puckery taste and also why it can bind dietary iron.
Ursolic acid in the flowering tops doesn't dissolve well in water, so a plain water tea pulls out far less of it than an alcohol-based tincture would.
The flavonol glycosides need their sugar removed before the remaining part can cross into gut cells, a step handled either by an enzyme on the gut lining or by colonic bacteria.
