- For adults wanting occasional, short-term help when things have stalled. Anyone after gentle everyday support is better matched to fibre or magnesium.
- How much to take

Psyllium works by holding water and adding stool bulk. Cascara sagrada's anthraquinone glycosides are converted by colonic bacteria into compounds that stimulate colonic motility and reduce water reabsorption. The two routes are independent, so combining them increases the total effect on transit. That also means the combination can overshoot into cramping and loose stool more easily than either alone.
Magnesium salts draw water into the bowel lumen osmotically. Cascara acts on the colonic wall instead. Stacking them raises the chance of watery stool and fluid loss, and repeated fluid loss is itself a route to potassium depletion. The pairing is worth flagging rather than recommending.
Anthraquinone laxatives increase stool water and speed transit, and both raise potassium losses through the gut. Low potassium in turn slows bowel smooth muscle, which can push a person toward using more laxative. Potassium status is the thing to watch during any extended use, not something to correct by adding more stimulant.
Calcium absorption depends on contact time with the upper small intestine and on active transport there. A stimulant laxative that shortens transit reduces that window. The interaction is about timing rather than chemistry, so separating intake by several hours is the practical response.
Non-heme iron is absorbed mainly in the duodenum and is already inefficient. Anything that speeds material past that segment lowers the fraction taken up. This is a mechanistic expectation from transit physiology rather than a measured figure for cascara specifically.
Cascara's cascarosides are inactive as swallowed. Bacterial beta-glucosidases in the colon cleave the sugar and release the active aglycones. That makes the composition of the gut flora a real determinant of how strongly a given dose acts, and it explains part of the person-to-person variability. Whether a specific probiotic strain raises or lowers that conversion has not been mapped.
Glycyrrhizin in licorice promotes renal potassium loss through mineralocorticoid-like activity. Stimulant laxatives promote faecal potassium loss. Used together over weeks the two losses add, and the combination is one of the classic ways potassium drops without an obvious single cause.
Slippery elm forms a mucilage that coats the gut lining, and it appears alongside cascara in older herbal bowel formulas on the reasoning that it softens the stimulant's edge. The rationale is traditional rather than measured. No controlled work has tested whether the mucilage changes cascara's effect.

Cascara bark contains compounds called cascarosides that pass through the small intestine essentially untouched, because the sugar attached to them blocks absorption there.
Bacteria in the colon break that sugar bond apart and convert the released compounds into anthrones, and those are the forms that actually act on the colon lining.
Those active compounds increase colon muscle contraction and reduce how much water and electrolytes get reabsorbed, so more fluid stays in the stool.
Since activation depends on gut bacteria, the effect kicks in only after material reaches the colon, rather than following any absorption from the stomach.
