Gravel root and dandelion root sit together in North American and European folk formulas built around urine flow. Both are used traditionally to increase urine output, so combining them is assumed in folk practice to push in the same direction, though nothing has measured whether the effects add. There is no controlled human work on the pairing, and anyone on a prescribed diuretic should raise it with their prescriber first.
Marshmallow root contributes mucilage, which folk practice added alongside gravel root for its coating quality. The rationale is textural and historical rather than pharmacological. Read it as a formulation habit with a long paper trail and no trial data.
Any preparation that raises urine volume also raises the amount of potassium and sodium leaving the body in that urine. Traditional diuretic herbs are used long enough at a time for this to matter. The size of the loss with gravel root specifically has not been measured, so the caution is directional rather than quantified.

Milk thistle often appears next to PA-containing herbs in blends on the assumption that it offsets hepatic load. Nothing has demonstrated that silymarin reduces pyrrolizidine alkaloid activation or the protein adducts that follow it. Adding milk thistle does not license a higher PA intake.

Gravel root is the root and rhizome of a North American plant also called Joe Pye weed, previously classified under a different genus name.
Plants in this group carry a class of alkaloids that can be toxic to the liver, and how much is present varies with plant part, harvest timing, growing region and how it's extracted.
These alkaloids get activated by a liver enzyme into reactive forms that bind proteins and DNA, which is why intake limits are set on the alkaloids themselves rather than on the plant material overall.
How much of these alkaloids ends up in the product differs between a water-based tea and an alcohol tincture, so two products from the same harvest can differ a lot in alkaloid content at the same labeled dose.
