No study gave these as a pair, so they are not in the card above. But the reason they belong together is settled biochemistry, not a guess, so it is worth knowing.
Standardised ginkgo leaf extract is defined by a dual specification: roughly 24 percent flavone glycosides and roughly 6 percent terpene lactones. The flavone glycoside figure is a fraction of that whole extract, not a separate ingredient taken alongside it. A label declaring flavone glycosides without the terpene lactone figure describes only half the specification. Reading the two numbers together is the only way to know whether a product matches the extract used in the clinical literature.
The ginkgo flavone glycosides are glycosylated forms of quercetin, kaempferol and isorhamnetin. Gut microbial and intestinal enzymes cleave the sugars, releasing the aglycones that are then absorbed and conjugated. This means a ginkgo flavone glycoside dose is functionally a delivery vehicle for those three flavonols. Taking supplemental quercetin alongside adds to the same absorbed pool rather than contributing something new.
When a flavonol quenches a radical it becomes a phenoxyl radical itself, and ascorbate can reduce that species back to the parent compound. This recycling is measurable in chemical and cell systems and is the standard rationale for pairing flavonoids with vitamin C. Whether the recycling changes any outcome in a person taking both has not been established. The chemistry is settled, the consequence is not.
The ginkgolide fraction of ginkgo extract antagonises platelet activating factor, and omega-3 fatty acids shift eicosanoid production in a direction that reduces platelet aggregation. Because flavone glycosides are supplied as part of a whole extract that also carries the ginkgolides, this additive effect applies to the product a person actually takes. It matters most around surgery, dental procedures, and alongside anticoagulant or antiplatelet medication. Anyone in those situations should raise the combination with their prescriber.
Nattokinase has documented fibrinolytic activity and ginkgo extract has documented antiplatelet activity through its terpene lactone fraction. The two act at different points in haemostasis, which means the combination is additive rather than redundant. This is a flag rather than a recommendation: the bleeding-risk direction is the reason to note it. No trial has measured the pair in people.
Garlic organosulfur compounds inhibit platelet aggregation in laboratory and human work, and standardised ginkgo extract does the same through a different mechanism. Stacking them is a plausible additive effect on bleeding tendency. It is the sort of combination that shows up unnoticed in a cardiovascular support stack. Flag it before any procedure involving bleeding risk.
The sugar moieties on ginkgo flavone glycosides must be cleaved before absorption, and a substantial share of that hydrolysis is done by gut bacteria carrying beta-glucosidase and rhamnosidase activity. This is why absorption of flavonol glycosides varies between people with different microbiota. Whether adding a specific probiotic strain changes that conversion for ginkgo glycosides has not been measured. The dependence on microbial conversion is established, the intervention is not.
Alpha-tocopherol terminates lipid radical chains inside membranes, then is regenerated by aqueous-phase reductants including ascorbate, which itself can be spared by flavonoids. The compartments differ, which is the basis for combining them rather than choosing between them. This network chemistry is well described in vitro. It should not be read as a demonstrated clinical benefit of the combination.
