While nattokinase has demonstrated fibrinolytic activity in vitro and in some animal studies, human studies are somewhat limited and require further validation. More research is needed to determine its efficacy and optimal dosage for various cardiovascular conditions.

Why these belong in the same formula. Each row says what the basis is, from settled biochemistry through to a trial that measured the pair.
Both come from fermented natto, but vitamin K2 is the cofactor for carboxylating clotting factors II, VII, IX and X while nattokinase acts on fibrin in the other direction. This is why many nattokinase preparations are deliberately depleted of vitamin K.
EPA shifts eicosanoid output toward the less aggregatory thromboxane A3 and lowers normal platelet aggregation, alongside a fibrinolytic enzyme acting on fibrin. The two effects add on the same physiology.
Ginkgolide B antagonises platelet activating factor and reduces normal platelet aggregation. Combined with a fibrinolytic enzyme the two push the same direction through different steps.
Garlic organosulfur compounds lower normal platelet adhesion and aggregation. Stacking that with fibrin degradation compounds an effect on ordinary clot formation.
Curcumin dampens thromboxane generation and platelet activation at higher intakes. That runs alongside a fibrinolytic enzyme rather than against it.
Bromelain has its own fibrin-degrading and platelet-modifying activity, so it overlaps directly with what nattokinase does. The combined load should be accounted for rather than assumed independent.
Gingerols inhibit thromboxane synthase, lowering normal platelet aggregation at higher intakes. The direction matches that of a fibrinolytic enzyme.
Salicin converts to salicylate, which dampens platelet cyclooxygenase activity. Combined with fibrin degradation the effects on normal clotting stack.
High-dose alpha-tocopherol interferes with platelet protein kinase C signalling and with vitamin K-dependent carboxylation. Both effects run in the same direction as a fibrinolytic enzyme.
Arginine is the substrate for endothelial nitric oxide synthase, which relaxes vessel walls, a mechanism separate from fibrin turnover. Circulation formulas combine the two because the routes do not overlap.
Dietary nitrate is reduced to nitrite and then nitric oxide, supporting normal vessel tone through the nitrate pathway rather than through fibrin. Nitric oxide also has its own dampening effect on platelet activation, which is worth noting alongside a fibrinolytic enzyme.
Monascus-fermented rice supplies monacolin K, which acts on cholesterol synthesis, while nattokinase is a fibrinolytic protease with a different target entirely. The cited four-month randomised placebo-controlled trial tested them as one supplement and reported changes in blood lipid measures, which are markers rather than events. Anyone combining them is stacking two separate mechanisms in one product, and the monacolin side carries its own interaction profile.
Both are bacterial serine proteases taken orally for systemic rather than digestive purposes, and both are degraded by gastric acid and pepsin without protection. Combining them stacks proteolytic activity and stacks the same delivery problem. There is no combination trial cited here, so the pairing rests on class pharmacology.
Lumbrokinase is a separate fibrinolytic enzyme preparation with the same intended target, fibrin. Using both raises total fibrinolytic activity in a way neither label accounts for. Bleeding tendency is the effect to watch, and this pairing has not been measured together in the material cited here.
Pine bark extract reduces platelet reactivity in human work, and nattokinase acts on fibrin from the other side of the clotting system. The combined direction is additive on bleeding tendency rather than cancelling. The cited review specifically notes that anti-atherosclerotic supplements carry bleeding considerations that stack.
Proanthocyanidins reduce platelet aggregation in ex vivo and human measures. Adding a fibrinolytic enzyme moves a different arm of haemostasis in the same overall direction. Flag it as an additive effect to monitor, not a benefit to advertise.
Resveratrol inhibits platelet aggregation in laboratory and some human measures, which is a separate mechanism from fibrin breakdown. The two together push haemostasis further than either alone. Confidence sits at promising because the human platelet data for resveratrol is inconsistent in magnitude.
Quercetin dampens platelet activation pathways at achievable intakes in some human work. Paired with a fibrinolytic protease the effects add rather than offset. The practical note is additive bleeding tendency, particularly around procedures.
The cited review places vitamin D, menaquinone and nattokinase together in the context of calcium and phosphate handling and vascular calcium deposition. Vitamin D governs calcium absorption; the menaquinone that natto fermentation also produces supports carboxylation of calcium-binding proteins. The link to the enzyme itself is contextual rather than direct, so this stays early.
Magnesium acts on vascular smooth muscle calcium handling and appears in blood-pressure-support formulas on that basis. Nattokinase is studied in the same formulation space through a haemostatic mechanism. Effects on blood pressure measures may add, which is worth knowing for anyone already at the low end.
Nattokinase is a protein, and lowering gastric pH with an acidifier increases pepsin activity and speeds its degradation. Taking a gastric acidifier alongside an uncoated enzyme works against the enzyme reaching the intestine intact. This is straightforward protein chemistry and argues for separating the two doses.
CoQ10 sits in the mitochondrial electron transport chain and supports normal cardiac energy metabolism, an entirely different mechanism from fibrin breakdown. The two appear together in vascular-support formulas without any interaction between them. Grounding is formulation practice, not a measured pair.

Nattokinase is a protein-cutting enzyme from the subtilisin family, made by a Bacillus subtilis strain while it ferments soybeans into natto.
In lab systems it cuts fibrin directly, and that's why its strength gets listed in fibrinolytic units rather than in milligrams of protein.
It's a protein, so stomach acid and pepsin chew it up. The delivery format decides how much intact enzyme ever makes it as far as the small intestine.
Fibrinolytic units and milligrams don't convert into each other. Two products showing the same milligram number can declare very different activity, so the units are the figure that actually describes the dose.
Where Nattokinase comes from.
The enzyme comes from the bacteria used to ferment soybeans into natto. After fermentation it is washed out with water, concentrated, and tested for how much fibrin-breaking activity it has, which is why labels show units instead of just milligrams. Makers then decide whether to leave in the vitamin K2 the same fermentation creates and whether to coat the capsule so stomach acid does not break the enzyme down.
