Lomatium root carries coumarins and related benzofuran constituents. Coumarin-rich botanicals are conventionally flagged for additive effects on platelet function when stacked with agents that already reduce aggregation, and EPA-rich fish oil does exactly that at higher intakes. The concern is directional and mechanistic rather than measured for this species. Anyone on anticoagulant or antiplatelet medication should raise the combination with their prescriber before using it.
Nattokinase acts on fibrin directly. Adding a coumarin-bearing root to that stack pushes in the same direction, and the combined effect has not been characterised. This belongs in the caution column, not the benefit column.
Garlic constituents reduce platelet aggregation in a dose-dependent way. Stacking that with a coumarin-containing botanical compounds an effect neither one was chosen for. Worth flagging ahead of any planned surgical or dental procedure.
Both quercetin and furanocoumarin-type constituents can inhibit CYP3A4-mediated metabolism at the intestinal wall, and inhibition from two directions is not simply additive.
