Both release free choline into the same pool used for acetylcholine and membrane phosphatidylcholine synthesis. Their choline contributions add, so a formula should count them together.
Citicoline donates the activated choline head group for phosphatidylcholine while DHA supplies the long-chain acyl tail. The two build different parts of the same membrane phospholipid.
Acetylcholine is choline plus an acetyl group taken from acetyl-CoA. Pantothenate forms the CoA backbone, so it supplies the half of the molecule citicoline does not.

The overlap is on nucleotide supply for phospholipid synthesis, a mechanism rather than a measured outcome.
Choline can be oxidised to betaine and used as a methyl donor, which places it in direct competition with the folate and B12 route for remethylation. Adequate B12 keeps the folate-dependent arm running so choline is spared for phospholipid work. This is cofactor logic, not a combination trial.
When folate supply is low, more choline is diverted through betaine to donate methyl groups. Supplying methylfolate reduces that pull, leaving choline derived from citicoline available for membrane synthesis. The relationship is a documented substitution between two methyl donors.
Choline kinase and CTP:phosphocholine cytidylyltransferase both act on ATP or CTP bound to magnesium. Without adequate magnesium those transfers slow, whatever the choline supply. This is textbook cofactor dependence and needs no combination study.
A published case report describes ultrapure citicoline given alongside homotaurine and vitamin E in an adult under ophthalmic monitoring, with the authors describing stable visual function measures across follow-up. One case cannot separate the contribution of any single component, and the measures reported are markers of function rather than a demonstrated effect. It documents the pairing rather than validating it.
The phosphorus spectroscopy work on citicoline reads out phosphocreatine and phosphodiester signals, the same compartment creatine loading acts on. Both therefore touch cellular phosphate handling, though from different directions: creatine buffers phosphate transfer while citicoline feeds membrane phospholipid turnover. No study has given them together, so this is mechanistic overlap only.
Citicoline donates phosphocholine to a diacylglycerol backbone, and the fatty acids on that backbone come from the diet. Long-chain omega-3 intake changes which species of phosphatidylcholine get built. The pairing is about substrate composition, not an additive effect on any endpoint.
Both are used for support of normal cognitive function and are frequently co-formulated. Ginkgo acts largely on vascular and platelet handling while citicoline feeds phospholipid synthesis, so the routes are distinct rather than reinforcing. No combination data exists.
