Notoginseng saponins and ginkgolides have each been described as reducing platelet aggregation in laboratory and animal work. Stacked together the effect is plausibly additive, which matters most for people already on anticoagulant or antiplatelet medication. This is a mechanistic expectation rather than a measured clinical interaction. Bruising or slow-stopping nosebleeds are the practical signal to watch.
Nattokinase is a fibrinolytic enzyme and notoginseng saponins have been reported to reduce platelet aggregation, so the two act at different points of the same process. The combination has not been characterised in controlled human work. Read this as a caution flag rather than a recommendation. Surgical procedures are the setting where the stack deserves a pause and a conversation with the prescriber.
High-dose long-chain omega-3s shift eicosanoid balance toward less aggregatory thromboxanes, and notoginseng saponins have their own reported antiplatelet signal. The two do not compete, they push in the same direction. For most people at normal intakes this is unremarkable. It becomes worth flagging when omega-3 intake is in the multi-gram range.
Garlic thiosulfinates inhibit platelet aggregation in laboratory preparations, as do several notoginseng saponins. Neither effect is large on its own at food or ordinary supplement intakes. The pairing is a stacking consideration rather than a benefit claim. Clinical confirmation of the combined effect is absent.
Dencichine, a non-protein amino acid in san-qi root, has been described as shortening bleeding time in animal work, while the saponin fraction has been described as doing the opposite. Vitamin K1 supports the carboxylation of clotting factors, an established and separate step. The net direction when the two are combined is unpredictable from mechanism alone. Anyone on warfarin should regard the whole combination as requiring monitoring rather than adjustment by guesswork.
San-qi and astragalus appear together in traditional formulas aimed at circulation and recovery. The pairing is documented by usage history rather than by controlled comparison of the pair against either herb alone. Their constituent classes differ, saponins in one and polysaccharides plus astragalosides in the other, so overlap is limited. Read this as formulation convention.
Dang gui and san-qi are combined in traditional practice for circulation-related uses. Dang gui carries coumarin-type constituents, which adds a second reason to be careful in anyone on anticoagulants. No controlled human data compares the pair against either component. The grounding here is usage history.
Piperine slows several phase I and phase II enzymes and P-glycoprotein, which raises systemic exposure to a range of co-ingested compounds. Whether this meaningfully changes notoginsenoside exposure has not been measured directly. If it does, it raises exposure to everything else in the formula too. The mechanism is well characterised even where the specific pairing is not.
Intact ginsenosides are large glycosides and poorly absorbed. Colonic bacteria strip sugar residues to yield compound K and related aglycone metabolites, which cross the gut wall far more readily. People differ substantially in how much of this conversion their microbiota performs. Whether a given probiotic product carries the deglycosylating strains is a product-specific question, not a general one.

Where San-Qi Ginseng comes from.
It is the root of a ginseng relative grown in southwest China, dried and sometimes steamed, then ground or extracted. Grading is traditionally by how many roots make up a given weight, so fewer, bigger roots means a higher grade.
