What Hexarelin is, and what it does.

Does it work
This is an investigational peptide studied in short pharmacology work, not a supplement ingredient. It belongs in research settings and under medical supervision.
How much to take
The band on record is 100 to 200mcg a day, and 400mcg is a research condition rather than a daily target. Repeat dosing shrinks the pulse as the receptor desensitises.
Time to feel it
A growth hormone pulse follows within roughly fifteen to thirty minutes of a dose, read on a blood assay. Nobody has established a timeline for anything a person notices.
The first dose
A growth hormone pulse peaks within about half an hour on a blood assay, usually with hunger soon after the dose. Cortisol and prolactin move alongside it.
With regular use
Repeat dosing blunts the hormone pulse as the receptor desensitises, and cortisol and prolactin move alongside it. Human data past a few weeks has not been collected.
How well tolerated
It moves several hormone axes at once, including cortisol and prolactin, so it needs medical supervision. Anyone subject to anti-doping testing should check its status first.
How it feels
Reported experiences are hunger shortly after a dose and sometimes warmth or flushing. Beyond short pharmacology studies, the subjective experience has not been catalogued.
The overlooked benefit
It also binds CD36 in heart tissue and macrophages, a site the other secretagogues in its class do not share, so its pharmacology is not interchangeable with theirs.

The largest natural growth hormone pulse occurs in early slow-wave sleep, so anything that changes sleep architecture changes the baseline a secretagogue acts on. Melatonin is used on that reasoning. The interaction is timing-related and has not been measured for this pairing.

What Hexarelin actually does.

Hexarelin is a synthetic hexapeptide of the growth hormone secretagogue class, structurally unrelated to growth hormone itself. It acts as an agonist at GHS-R1a, the receptor whose endogenous ligand is ghrelin, so it prompts release of stored hormone rather than supplying any.

GHS-R1a activation on pituitary somatotrophs works through the Gq pathway, raising inositol trisphosphate and intracellular calcium, which triggers release of growth hormone granules as a pulse rather than as a steady output.

Somatostatin tone from the hypothalamus opposes that release, which is why the size of the response to a secretagogue depends on when in the natural rhythm it is given and on the prevailing somatostatin signal.

As a six-residue peptide it is cleaved by gastric acid, pepsin and brush-border peptidases, so oral exposure is a small fraction of parenteral exposure. The C-terminal amide and the D-amino acid substitutions in its sequence are there to slow, not stop, that degradation.
