Tempoyak is durian pulp fermented by lactic acid bacteria, so the food itself carries live organisms alongside the fruit's fibre. A 2026 review positioned it as a traditional probiotic source and discussed microbiome modulation. That is a review of a fermented food, not a trial of a durian supplement.
The lactic acid bacteria that drive tempoyak fermentation are largely Lactobacillus species, with plantarum prominent among them. They acidify the pulp, change its sugar profile and generate organic acids. This is food microbiology rather than a demonstrated benefit in people.
Durian pulp extract was reported to raise intracellular glutathione in cell work, and N-acetylcysteine raises glutathione by supplying the limiting cysteine. Both point at the same pool from different directions. The durian side of that pairing is cell culture only, so read the combination as mechanistic.

Condensed tannins bind non-heme iron and form complexes that the intestine absorbs poorly. Procyanidin rich plant material eaten in the same meal as an iron supplement reduces the iron actually taken up. Spacing the two by a couple of hours is the standard workaround.
Durian carries a high potassium load per serving relative to most fruit. Anyone already taking a potassium supplement, or on medication that raises serum potassium, is adding to the same total. That total is what matters, not the fruit in isolation.
