Berberine, the main Coptis alkaloid, is pumped back into the gut lumen by P-glycoprotein, which is why its oral uptake is low. Piperine inhibits that pump and raises systemic exposure.
Piperine blocks intestinal P-glycoprotein and slows first-pass metabolism of berberine-type alkaloids. Coptis is routinely paired with it for that reason.
Long pepper supplies the same piperine that inhibits the efflux pump limiting berberine uptake. The pairing raises the alkaloid exposure from a given Coptis dose.
