Women's formulas typically carry iron, and iron shares divalent transporters with zinc and calcium in the same tablet. Adding more iron on top means counting the tablet's amount first.
Ascorbate reduces ferric to ferrous iron and holds it soluble, raising uptake of the iron in the same serving. This is why iron-containing multivitamins carry vitamin C in the base.
Gram-level calcium in the same serving competes with the tablet's iron and zinc for shared transporters. Calcium is normally taken at a separate time.
Magnesium is bulky and a single tablet can only hold a fraction of the daily requirement. A separate magnesium dose is the standard companion.
Women's multivitamins carry folate or folic acid as standard, so a separate folate dose stacks on an amount already in the tablet. Read the tablet before adding.
Vitamins A, D, E and K need dietary fat and mixed micelles for absorption, which an oil taken alongside supplies. Long-chain omega-3s are also missing from tablet multivitamins.
A multivitamin's vitamin D amount is usually a baseline well under common intakes. Extra D3 raises the total without duplicating the rest of the tablet.
Vitamin D raises calcium absorption while vitamin K dependent proteins direct where it is deposited. Multivitamins rarely carry meaningful MK-7, so it is the usual addition.
Choline requirements run to hundreds of milligrams, too much to fit in a multivitamin tablet. It is added separately when it matters.
The zinc in a multivitamin induces intestinal metallothionein, which binds copper and lowers its absorption. Copper is paired in for that reason.
Zinc, iron and copper all pass through the divalent metal transporter DMT1 at the brush border, so a large single dose of one reduces the uptake of the others taken with it. Sustained high zinc also induces metallothionein in enterocytes, which binds copper and holds it in the shed cell. A multivitamin balances these deliberately at modest doses; stacking a separate high-dose single mineral on top is what changes the arithmetic.
Methionine synthase needs both 5-methyltetrahydrofolate and B12 as methylcobalamin to remethylate homocysteine. Folate without adequate B12 leaves that enzyme without its cofactor. This is why the two are formulated together in every credible multivitamin rather than either one alone.
Pyridoxal-5-phosphate is the cofactor for cystathionine beta-synthase, the enzyme that routes homocysteine down the transsulfuration branch toward cysteine. Folate and B12 handle the remethylation branch. Between them the three B vitamins cover both exits from homocysteine, which is why they travel as a set.
MTHFR, the enzyme that produces 5-methyltetrahydrofolate, uses FAD derived from riboflavin as its cofactor. Riboflavin status therefore sets how well dietary folate is converted to its active circulating form. It is the least discussed member of the folate cycle and the one most often left out.
The deiodinase enzymes that convert thyroxine to the active triiodothyronine are selenoproteins, and glutathione peroxidase is another. Iodine supplies the hormone backbone and selenium supplies the enzymes that act on it. A multivitamin carrying iodine without selenium leaves one half of that pair unsupported.
Iodine is structurally incorporated into thyroid hormone; there is no substitute for it in that molecule. Its handling depends on selenium-containing enzymes downstream. Requirements rise during pregnancy and lactation, which is why women's formulas are the ones that most often carry it.
Biotin is the covalently bound cofactor of the four human carboxylases involved in fatty acid synthesis, gluconeogenesis and amino acid catabolism. High supplemental biotin also interferes with streptavidin-biotin immunoassays used for thyroid and cardiac blood tests, which is a laboratory artefact rather than a physiological effect. Anyone having bloods drawn should mention biotin intake.
Manganese uses DMT1 and transferrin, the same routes as iron, so iron status and manganese uptake move in opposite directions. Low iron status increases manganese absorption. In a multivitamin both are present at modest amounts, which is the setting where this balance was worked out.
Molybdenum sits in the molybdopterin cofactor of sulfite oxidase, xanthine oxidase and aldehyde oxidase. Requirements are in the tens of micrograms, which is why it appears in trace amounts or not at all. High copper intake and molybdenum interact in the gut through thiomolybdate formation.
The vitamin D receptor works as a heterodimer with the retinoid X receptor, so retinoid status shapes vitamin D signalling at the level of gene transcription. Preformed vitamin A as retinol also has a defined upper intake limit that matters specifically before and during pregnancy. Beta-carotene is converted on demand and does not carry the same ceiling.
Alpha-tocopherol stops lipid peroxidation chains in the membrane and is left as a tocopheroxyl radical, which ascorbate regenerates back to the active form at the membrane surface. The two work as a redox pair across the water and lipid boundary. High-dose alpha-tocopherol alone also displaces gamma-tocopherol, which is why mixed tocopherols exist as a separate form.
Vitamins A, D, E and K need bile salts and dietary fat to form the mixed micelles that carry them across the intestinal wall. A multivitamin taken with a fat-free drink delivers less of its fat-soluble fraction than the same tablet taken with a meal. Any fat source serves this purpose; MCTs are simply the one most often formulated in.
Boron has been reported to reduce urinary calcium and magnesium losses and to influence circulating steroid hormone levels in small human studies. There is no established human enzyme requiring boron, so its role stays descriptive rather than mechanistic. It appears in bone-oriented women's formulas on that basis.
Chromium is a standard trace component of multivitamin and mineral formulas, included for its association with normal glucose metabolism. In a multivitamin it sits alongside minerals that compete with it for absorption at high single doses. Multivitamin amounts are modest enough that this stays a formulation consideration rather than a practical problem.
Tea polyphenols bind non-heme iron in the gut lumen and form complexes that are not absorbed, an effect measured for ordinary tea drinking as well as for concentrated extracts. A women's multivitamin usually carries iron, so taking it with a green tea extract or with tea itself reduces the iron actually taken up. Separating them by an hour or two removes the issue.
Piperine inhibits intestinal glucuronidation and P-glycoprotein efflux, which raises circulating levels of several co-administered compounds. That is a general effect on absorption, not a nutrient-specific one, and it applies to medications as much as to nutrients. Its presence in a multivitamin is a formulation decision worth knowing about.
A randomised study supplemented fish oil with and without a multivitamin and measured omega-3 incorporation into red blood cell membranes. Membrane fatty acid content is a marker of intake and distribution, not a health outcome. The two products are commonly taken together because their nutrient coverage barely overlaps.
Coenzyme Q10 is synthesised endogenously from tyrosine with B vitamins as cofactors along the pathway, so B-vitamin status sits upstream of the body's own CoQ10 production. Supplemental CoQ10 enters the same membrane pool that vitamin E protects. The pairing is mechanistic reasoning rather than a measured combination effect.
Myo-inositol is the backbone of the phosphoinositide second messengers used in insulin and gonadotropin signalling, and it is one of the more studied additions to women's formulas. It is made endogenously from glucose and also supplied by diet. Supplemental amounts used in trials are grams, far above what a multivitamin tablet can carry.
