On an 8-hour metabolic study day, healthy adults took a single 20 mL dose of tricaprylin (C8), tricaprin (C10), trilaurin (C12) or a mixed C8/C10 oil with breakfast, with a second dose four hours later. The rise in plasma acetoacetate, beta-hydroxybutyrate and total ketones was largest after C8 and occurred 0.5 to 3 hours after the dose. In a separate study, 10 healthy adults given single 10, 20 or 30 g doses with breakfast and sampled every 30 minutes for four hours showed roughly a two-fold rise in ketogenesis over the no-treatment control. In a randomised crossover in 8 lean adults and 8 adults with obesity, ketogenesis and metabolic rate rose and blood glucose fell over five hours, and the same response was present after eight days of daily intake. Blood ketones were measured, not alertness.

The caprylic acid in a C8 oil is a medium-chain fat that passes into the mitochondria and is burned for energy without the carnitine shuttle, whereas long-chain fats depend on carnitine to get in. Carnitine covers the long-chain route while C8 feeds the carnitine-free one, so the two support fat burning through separate doors rather than one speeding up the other.
Coenzyme Q10 is a large oily molecule that barely dissolves in water, so taken dry only a small share of a dose is picked up from the gut. Dissolved into a lipid such as C8 oil it stays in the fat phase through digestion and rides the fat droplets the intestine absorbs, which raises the fraction that reaches the bloodstream.
Curcumin dissolves readily in fat but poorly in water, so on its own most of a dose passes through unabsorbed. Taken dissolved in a lipid such as C8 oil, more of it stays solubilized in the fat phase as it moves through the gut and crosses the intestinal wall.
Cholecalciferol needs a lipid phase to be micellised and absorbed. A caprylic acid base is the usual carrier in softgels and drops for exactly that reason.
MK-7 is a long-chain lipophilic quinone whose uptake depends on dietary fat. Dissolved in a medium-chain oil it enters the same micelles reliably.
Retinyl esters are absorbed with dietary lipid after hydrolysis at the brush border. An oil base supplies that lipid.
Tocopherol needs fat for absorption and in turn protects the oil phase from peroxidation. The relationship runs both ways in one softgel.

Medium-chain triglycerides are hydrolysed rapidly and their fatty acids are absorbed into the portal vein without needing micelle formation, so they depend far less on pancreatic lipase and bile than long-chain fats do. A lipase supplement therefore adds little to C8 handling specifically. This is a reason not to expect a benefit, which is as useful as a reason to expect one.
Leucine is a ketogenic amino acid whose carbon skeleton reaches acetoacetyl-CoA, the same node C8 oxidation feeds. Both therefore contribute to the hepatic acetyl-CoA and ketone body pool. The size of the contribution from ordinary leucine intake is small relative to a C8 dose.
Phospholipid emulsifiers are used to disperse C8 oil into water-based drinks and powders, keeping it from separating and reducing the throat burn of the neat oil. This is a delivery function rather than a metabolic one. Stated as formulation practice.

Medium-chain fatty acids released from C8 triglyceride are absorbed into the portal vein and reach the liver directly, rather than being packaged into chylomicrons and entering via lymph as long-chain fats are.
C8 enters the mitochondrion without the carnitine palmitoyltransferase shuttle that long-chain fatty acids require, which is why its oxidation is fast and relatively independent of carnitine status.
Rapid hepatic beta-oxidation of C8 generates acetyl-CoA faster than the liver's citric acid cycle consumes it, and the surplus is condensed into acetoacetate and beta-hydroxybutyrate.

Both oils are naturally rich in medium-chain fatty acids, with coconut and palm kernel the only two commodity fats that carry enough C8 to make isolation worthwhile. Which one was used is a sourcing and land-use question, and it is frequently absent from the label.
The triglycerides are split so the individual fatty acids can be separated by chain length; glycerol is recovered from this step and often sold onward.
Fatty acids are separated by boiling point under vacuum. The C8 fraction is drawn off from C6, C10, C12 and longer chains, and how tightly that cut is drawn is what determines whether a product can be labelled C8 only.
