In a randomised trial, 48 participants were allocated to tart cherry or placebo as juice or as powdered capsules, once or twice daily over 48 hours, with serum uric acid, high sensitivity C reactive protein and plasma oxygen radical absorbance capacity tracked. Serum uric acid fell by an average of 8 percent, declining for up to 24 hours after a single capsule dose, up to 8 hours after two capsules and up to 2 hours after a single juice dose, and the authors describe the fall as transient and not maintained with continued supplementation. Neither hsCRP nor oxidative capacity changed. A later crossover in 25 adults with mildly raised uric acid found no effect on the uric acid response to a high purine meal.

The colour compounds are anthocyanins, principally cyanidin-3-glucosylrutinoside and cyanidin-3-rutinoside; they are absorbed poorly as intact glycosides and appear in circulation mostly as protocatechuic acid and other small phenolic metabolites produced by gut bacteria.

Flavonoids from the fruit are conjugated within minutes by UDP-glucuronosyltransferase and sulfotransferase enzymes in the gut wall and liver, so plasma concentrations of the intact compounds are low and short-lived by design of the system, not by failure of the product.
