In an eight week randomised, double blind, placebo controlled trial, 101 adults with knee osteoarthritis took 500 mg of a standardised curcumin extract twice daily or placebo. Knee pain scores on the KOOS and on a numeric rating fell more than placebo across the eight weeks, and timed up and go and six minute walk results also moved. One author was affiliated with DolCas Biotech, which makes the extract tested. A separate 28 day open label trial in 139 adults measured pain at days 7, 14 and 28 against diclofenac rather than against placebo, and found no significant difference between the two.

Can I just eat more curry?
You'd have to eat a ton. Curry powder is only 2-5% curcumin and it's poorly absorbed. Supplements are way more potent and targeted.
Why does it need black pepper?
Black pepper has piperine, which boosts curcumin absorption by up to 2,000%. Without it, most of the good stuff just passes right through you.
Is it the same as curcumin?
Almost. Turmeric is the whole root. Curcumin is the main active compound inside it that does the anti-inflammatory work. Good supplements concentrate the curcumin.
Can I take it instead of Advil?
Different tools for different jobs. Advil is for fast, acute pain relief. Turmeric is for managing long-term, chronic inflammation. It's not a painkiller.
How long until I see results?
Be patient. Give it at least 4-6 weeks of consistent daily use. Some people notice it sooner, but it's definitely not instant.

Piperine, the active compound in black pepper, slows the gut and liver enzymes that rapidly attach glucuronic acid to curcumin and clear it, so more curcumin stays in circulation. This is the reason the two are so often put in the same capsule.

Chondroitin is a large sulfated polysaccharide with limited oral absorption and curcuminoids are small lipophilic phenolics. Their absorption routes do not intersect. They sit together in joint formulas for separate reasons. Regard the pairing as convention.

MSM is small, highly water-soluble and almost completely absorbed, the opposite profile to a curcuminoid extract. In a powder blend its solubility helps wet a hydrophobic extract during mixing. Their roles in a joint formula are independent. No combination trial exists.

Hydrolysed collagen supplies glycine and proline-rich peptides and also serves as a bulk matrix that a small amount of turmeric extract disperses into in a drink mix. Oral hyaluronic acid is depolymerised by gut bacteria before any absorption, a completely different route from the micellar uptake curcuminoids depend on. The two coexist in joint and skin formulas without interacting. Convention rather than chemistry. No combination data exist.

What Turmeric Extract actually does.

Turmeric root itself is only about two to five percent curcuminoids, while a standardised extract is concentrated to around ninety five percent. So the same weight of extract and of plain root powder don't give you anything close to the same amount.

What a label calls curcuminoids is really three related compounds: curcumin plus its two close cousins, demethoxycurcumin and bisdemethoxycurcumin. They differ only by small chemical side groups.
